Description: Betty, an 88-year-old woman with mild cognitive impairment, is brought to the ED with 48 h of non-radiating, central abdominal pain and anorexia. Her bowels last opened 3 days ago and she has not had a fever. Past medical history includes atrial fibrillation, hypothyroidism and an appendicectomy. On examination, she is afebrile, has a BP of 162/87 mmHg, HR of 96 BPM, normal oxygen saturation and respiratory rate. Her abdomen is soft with no peritonism but has central and right-sided tenderness. Rectal examination reveals an empty rectum.
